USING ONLINE DOCUMENTS
Basic Online Document Information

1. All sample letters and forms must be printed and submitted on municipal letterhead.
   
   
2. Matter capitalized and in brackets, designated [THUS], should be replaced with verbiage specific to the municipality or the specific emergency.
   
   
3. Submitted documents must be signed by the person indicated. Documents will not be accepted without complete information and authorized signatures.
